Is the THB 20,000 cap total or per case?

Per departure (per case). 41 days × 500 = 20,500 → pay only 20,000. But arrest inside Thailand has no cap.

Is self-reporting at Immigration better than at airport?

For longer overstays it usually is. The fine amount is identical, but settling at an Immigration office produces an official receipt and a documented voluntary disclosure, which is useful evidence if a future entry or visa application is questioned. Short overstays are routinely cleared at the airport counter.
